Make-up Classes:

• Due to limited space and pool time we do not offer make up lessons for missed classes

Waiver:

Every precaution will be taken to ensure you and your child’s safety. However, we request that you read and sign the following prior to participation.

I hereby give permission to have staff arrange for any emergency medical care including hospitalization if necessary. In all instances attempts will be made to contact a parent or guardian first. The participant is responsible for his or her own medical coverage unless they are under the age of majority, then the parent or guardian is responsible for the participants medical coverage. The undersigned hereby releases and forever discharges Back To Basics from all claims and liabilities whatsoever which may arise in connection with the operation of the swim program or arising from any participation by the undersigned and/or children of undersigned in any activity offered by Back To Basics save and except from the negligence or willful misconduct of Back To Basics or those in law from whom Back To Basics is responsible. Back To Basics is not responsible for lost or stolen articles.
